The MT-137 is used extensively by U.S. military units, fire and EMT personnel, as well as a large number of federal, state and local response agencies, including the Department of Defense, Department of Corrections, and state and local Emergency Management Agencies and Operations Centers.
In the event of a Mass Casualty Incident (MCI), where multiple victims are injured and require varying degrees of medical attention, it is absolutely imperative that first responders have a quick, easy, and reliable method of assessing victims and assigning them with an appropriate triage or priority status, based on the severity of their injuries. The MT-137 Triage Tag has been tested and proven for over 40 years to provide this method.

Additionally, each Medical Emergency Triage Tag is equipped with a 30-inch elastic band, which is used for attaching the tag to the victim's body, usually around the neck or to an available limb.
Each MT-137 is coded with a unique sequential barcode serial number, which aids in the identification and tracking of victims and their belongings through the various phases of the triage and treatment processes, both in the field, as well as into the receiving hospital emergency care facilities. These unique serial numbers are conveniently located on both the top right and top left diagonal tear-offs, as well as on the lower color coded tear-offs.
By utilizing standard graphic symbols and a four-color tear-off triaging system, the Standard Medical Emergency Field Triage tag (MT-137) is truly a universal tool, in that it presents no language or literacy barriers, and can be used in the field by anyone, of any nationality, with minimal training or hands-on experience. A policy of using METTAG™ products in day-to-day emergencies will automatically establish familiarity and provide faster and more effective victim triaging in the event of an actual MCI or other emergency.

The MT-501 was developed by TACDA/METTAG in conjunction with Gold Cross Ambulance and follows the triaging system guidelines suggested by the NHTSA (The National Highway Traffic and Safety Administration.) The MT-501 integrates the S.A.L.T. algorithm which is Sort, Assess, Life-saving interventions, and Treatment/Transport in a simple and quick use format.
- Sort - Sort the walking wounded, those calling or waving for help, and those that are not moving.
- Assess – Assess to find life-threatening injuries. Note findings with simple yes or no answers.
- Life-Saving Interventions – Give necessary interventions to save life. Apply tourniquets, open airways, give breaths, and administer medications, within one minute of time. Tear off the perforated sections at the bottom of the tag indicating patient level of triage.
- Treatment/Transport – After assessing patients and giving life-saving interventions, transport patients according to urgency of need by noting color coded tear offs at the bottom of the tag which are dead, immediate, expectant, delayed and minimal.

S.T.A.R.T. Method Triage Tag

METTAG's S.T.A.R.T. Triage Tag follows the recommended triage approach S.T.A.R.T. developed by Newport Beach Fire and Marine Department in 1983. S.T.A.R.T. stands for "Simple Triage And Rapid Treatment." Simple triage and rapid treatment (START) is a triage method used by first responders to quickly classify victims during a mass casualty incident (MCI) based on the severity of their injury.

History of the START Tag courtesy of the National Library of Medicine:
START was developed by the Newport Beach Fire and Marine Department and Hoag Hospital in Newport Beach, California in 1983. Initially it used the ability to obey commands, respiratory rate, and capillary refill to assign triage category. Modifications to START in 1996 by Benson et. al. substituted radial pulse for capillary refill, with a report of improved accuracy, especially in cold temperature. The Benson revision (START - SAVE [Secondary Assessment of Victim Endpoint]), also incorporates additional factors that determine "survivability" over time as the event progresses and assumes limited response resources. There has been limited rigorous scientific review of various forms of mass casualty incident triage used around the world. New methods of triage using new algorithms have been proposed, but not tested in the field. At present START remains the most used mass casualty triage algorithm in the US.

Development:
Efficient and effective triage during Mass Casualty Incidents (MCI) has proven to save lives. MET-TAG Medical Emergency Triage Tags have historically been used worldwide in MCIs ranging from automobile and aircraft accidents involving several victims, to large-scale terrorism incidents. With rising concerns of how to effectively handle triage situations involving HAZMAT, terrorism, and possible use of weapons of mass destruction, the need for an alternative triage tag has emerged. The CB-100 Chemical/Biological triage tag was designed to meet the needs of the first responders to the growing complexities and dangers of the modern triage environment.
Construction:
Constructed of the same high-density water resistant synthetic paper and printed using the same special thermal printing process as the Original MT-137, the front of the CB-100 Chemical/Biological MET-TAG has all of the key information fields needed to quickly note the triage patient’s status and to fill in additional patient assessment and tracking information as time permits. The back of the CB-100 provides room for additional notes about the patient, if needed. Each section of the CB-100 has the unique barcode serial number for tracking and monitoring the victim’s triage process.
Instructions for Use:
- The top portions of the tag can mark a patient’s original location (as required in MCIs involving aircraft, etc.), and can track a patient’s personal articles and/or body parts that may have been severed in an MCI.
- Other tear-off tabs near the top of the CB-100 are to be used for the Incident Commander or Triage Unit Leader’s log as well as to track how, when, and where a patient was transported for further treatment.
- The middle sections provide the receiving hospital or critical care facility with a permanent record of the patient’s personal information, medical history, vital signs, and initial triage status. This middle section also provides a strip for the patient to keep, in order to help them track down their personal property, and will help to expedite triage actions if a first responder is injured while assisting victims at an MCI (first responders can fill in information about themselves prior to an actual MCI, such as medical history, name, contact information, etc., and then wear their own CB-100 when entering the scene).
- The bottom portion of the CB-100 has the standard tear-offs to color code the triage status of the patient. In addition, just above these tear-offs is a Triage Status Log to show if a victim's/patient’s triage status changed during the course of an MCI.
The CB-100 can be used with standard triage systems like the START method or with the A-E Triage System that is specifically designed to handle HAZMAT and terrorist incidents.
